Persvorm van messing in twee delen voor tabakspijp met ovale ketel, hiel en rechte steel, zogenaamd verdrietje. Onversierd. Hiel links ingeslagen formaatcijfer "12". Vormhecht zes ijzeren sluitpennen, kroon twee ijzeren stuurpennen.
